Composite Bonding

Composite bonding is a conservative, cost-effective aesthetic treatment in which composite material is layered onto the tooth surface with no or minimal enamel reduction. It is usually completed in a single session.

It delivers excellent results for closing gaps (diastema), repairing chipped or cracked teeth, lengthening short teeth and correcting mild shape or colour irregularities. It is a faster, more affordable and less invasive alternative to porcelain veneers.

At our clinic, we use nano-hybrid composites that closely mimic the translucency and fluorescence of natural enamel. The dentist sculpts the material in layers, cures it with a special LED light and finishes it with discs and rubber points to a mirror-like polish.

A major advantage is reversibility: if you later choose a more permanent treatment such as porcelain veneers or crowns, the underlying tooth structure is preserved.

What Composite Bonding Is

Composite bonding is the shaping of a tooth-coloured composite resin directly on the tooth. Nothing made in a laboratory is bonded on; the material is applied by the dentist in layers, shaped, cured with light and polished.

It is therefore one of the procedures in which manual skill shows most directly. The same material produces very different results in two different hands. What decides the outcome is not the brand but the person doing the layering and the polishing.

Modern composites come very close to the light transmission and fluorescence of natural enamel. Composites of different opacities and shades are layered to reproduce the depth of a tooth; a restoration made from a single shade looks flat and matt.

Its greatest advantage is that it is reversible. Placed additively, almost no tooth structure is removed; when it is taken off, the enamel underneath is as it was, allowing for the few microns lost to etching and polishing.

That is what separates it most clearly from a laminate veneer: bonding is not a decision so much as a reversible trial.

What It Solves

Bonding is designed for small and medium-sized aesthetic problems. Within its limits it is an excellent solution; pushed beyond them it is short-lived.

  • A chipped or worn incisal edge. Repaired in one appointment; the most common bonding case.
  • A gap between teeth (diastema). Particularly the space between the central incisors, closed by adding to both.
  • Short or disproportionate teeth. Undersized lateral incisors are common and are corrected with bonding.
  • Mild discolouration and spots. Limited marks that do not respond to whitening can be covered.
  • Old, discoloured front fillings. Replacing them is bonding work.
  • Covering exposed root surface. For sensitivity and appearance after recession.
  • Minor shape irregularities. A canine that looks too pointed, an uneven edge.

What it does not solve is equally clear: substantial loss of structure, root-treated and weakened teeth, significant malocclusion and severe crowding. Those need a crown, a veneer or orthodontics.

There is also a "possible but not correct" group: bonding applied to many teeth in thick layers. Rebuilding the shape of eight teeth in composite is technically possible, but the result looks bulky, stains faster and is harder to maintain. In that case porcelain is the better answer.

What Happens in a Single Appointment

  1. Shade selection. Done before the tooth dries, in natural light. Skip this first step and the result stands out however well it is shaped.
  2. Surface preparation. The tooth is cleaned and the enamel etched. No reduction is carried out; etching creates a bonding surface at a microscopic level.
  3. Adhesive. The bonding agent is applied and light-cured.
  4. Layering. Composite is placed in layers of differing opacity, each light-cured. The depth of the tooth is built here.
  5. Shaping. The outline, the surface texture and the incisal translucency are created.
  6. Occlusal check. Biting and lateral movements are checked; composite taking excessive contact is the commonest cause of it fracturing.
  7. Polishing. The surface is polished with graded discs and pastes. This step directly determines stain resistance and must not be rushed.

A single tooth takes roughly thirty to sixty minutes. Two to three hours is set aside for four front teeth. This is the most misunderstood point about bonding: "one appointment" does not mean "a short appointment".

Bonding is work measured in time, and when it is rushed the first thing lost is the polish. Inadequately polished composite goes matt within months and begins to pick up stain. A clinic running a fixed package schedule cannot give three hours to four teeth.

Anaesthesia is usually not needed. Because no tooth is cut the procedure is not uncomfortable; local anaesthetic is used where root surface is being covered or where there is decay under an old filling.

Bonding or a Veneer?

These two are often confused, and the difference is the most important decision the patient makes. It is not a difference in quality but in permanence and reversibility.

A practical rule for deciding: if an additive solution is enough, bonding; if there is lost structure or a permanent colour problem, a veneer. In younger patients and in uncertain cases, trying bonding first is correct — if it is not liked it comes off, and if it is, it serves for years before moving to porcelain if ever needed.

The reverse is not available: on a tooth already prepared for a veneer, bonding is no longer adequate. The sequence runs one way, and that tells you which to start with.

The commercial reality, stated plainly: bonding is among the least profitable things a clinic can propose. There is no laboratory invoice, it finishes in one appointment and it costs less. So proposing eight veneers where bonding would have done is a common pattern, and patients have a right to know that.

Here bonding is recommended wherever it is appropriate, and the difference shows in our invoice. We write that as correct treatment rather than as a virtue.

Staining and Aftercare

Composite stains more readily than porcelain. We do not hide that, because it is something the patient needs to know and to maintain accordingly.

Staining happens in two places: on the surface and at the margin. Surface dulling develops over time — the polish wears and the roughened surface begins to hold colour. The margin is the fine line between composite and tooth, and it can darken over the years.

The good news is that neither is permanent. At a routine appointment the composite is re-polished and most of the gloss returns; where the margin has darkened it is refreshed. That is why six-monthly check-ups are particularly valuable for a bonding patient.

  • Reduce staining drinks; take coffee, tea and red wine through a straw and follow with water.
  • Know that smoking stains composite faster than anything else.
  • Avoid abrasive whitening toothpastes — they damage the polish and accelerate staining.
  • Brush twice daily and clean between the teeth every day.
  • A check-up and polish every six months.
  • No biting hard objects, no nail-biting, no opening packaging with your teeth.
  • A night guard if you grind.

When a composite restoration chips or lifts at an edge, it is repaired in the mouth in most cases. That is a real advantage over porcelain, where the same situation usually means remaking the unit.

Like every treatment carried out here, composite bonding carries a lifetime guarantee on materials and workmanship. One distinction is written plainly: the guarantee covers workmanship and materials; the natural dulling and staining of composite over time is not a defect but a known property of the material, and it is handled with routine polishing.

Diastema: The Gap Between the Front Teeth

The gap between the upper central incisors is one of the most common reasons patients come for bonding, and also the one requiring most care.

There are two ways to close it. Orthodontics moves the teeth together; the gap disappears and the proportions of the teeth do not change. Bonding adds to both teeth; the gap closes but the teeth become somewhat wider.

That second point matters and is usually skipped. A wide gap closed with bonding alone can leave two incisors that look disproportionately broad. As a rough measure: gaps up to about two millimetres can be closed naturally with bonding; wider than that, orthodontics should be assessed first.

In some cases both are used together, and that gives the best result: orthodontics closes most of the space and bonding completes the remaining millimetre, keeping the proportions intact.

The cause also needs looking at. A labial frenum extending down between the teeth can prevent the gap closing and cause it to reopen after treatment. Those cases need a small surgical procedure. Closing a gap without looking at the cause means the same gap returns a year later.

Who Bonding Suits

Bonding suits a wide range of patients, but a few concrete conditions decide suitability.

  • Sound enamel is needed. Composite bonds very well to enamel and less well to dentine. Where most of the surface is enamel, the result lasts longer.
  • The gums must be healthy. Margins seal poorly in inflamed tissue and the restoration stains early. Active gum disease is treated first.
  • The bite must be suitable. Where the lower teeth contact the back of the upper incisors heavily, added composite is under constant load and fractures. The occlusion is assessed first in those cases.
  • A night guard is needed if you grind. Bonding without one is short-lived in a bruxist.
  • Expectations must be realistic. Bonding does not give the long-term surface quality of porcelain; in exchange, it preserves your tooth.

On age, bonding is particularly appropriate for younger patients, and that matters. Permanently cutting the front teeth of a young person starts a replacement cycle that will run for the following sixty years. Bonding leaves the tooth as it is and defers the decision.

The same logic applies to a patient of any age who is unsure what they want. Bonding is the reversible way to see the result: keep it if you like it, remove it if you do not, and no tooth has been lost.

How Long It Lasts

Composite bonding is not permanent and that should be said at the outset. What determines how long it lasts is the conditions rather than the material.

What happens in practice is that composite does not fail suddenly; it ages gradually. First the surface gloss reduces, then the margin becomes visible, and finally a small edge chip appears. Every one of those stages can be caught at a check-up, and most are repaired in the mouth.

That repairability is bonding's most valuable property. In porcelain a small fracture usually means remaking the unit; in composite the same situation is dealt with in a short appointment.

Like every treatment carried out here, composite bonding carries a lifetime guarantee on materials and workmanship. What it covers and what falls outside it — accidental damage, neglect, missed recommended check-ups, not wearing a prescribed night guard, and the natural ageing of the material — is set out in writing.

If You Are Travelling for Treatment

Bonding is the easiest treatment for a patient travelling for care, and there are concrete reasons.

There is no laboratory stage. No impression, no waiting for a try-in, no courier time. Because the work is done directly on the tooth, it finishes on the day it starts. One appointment for one or two front teeth; a long but still single appointment for four.

No temporary restoration is needed. That is a real difference: veneer and crown treatments involve days in temporaries and a second appointment; bonding does not.

There is no restriction on flying. Composite cures instantly under light and reaches its full strength as soon as the appointment ends; you can fly the same day. We only suggest avoiding very hard and sticky food for the first day.

There is also this: bonding is easily added to the plan of a patient who came to İstanbul for something else. A chipped edge on the front tooth of someone here for an implant or root canal treatment is repaired in an hour between appointments and needs no separate visit.

Review and polishing can be done where you live; no special training is needed and any dentist polishes composite. After treatment you are given the brand and shade code of the composite used, in writing — so that if a repair is needed later, the shade is not matched by guesswork.
