Dental Bridge

A dental bridge is a fixed prosthesis treatment that replaces one or more missing teeth with support from adjacent healthy teeth. Bridges prepared from high quality materials such as zirconium or E-max offer both an aesthetic and durable solution. You regain your chewing function and preserve the aesthetics of your smile. What a Bridge Is and How It Works

A dental bridge is a fixed prosthesis that closes the space left by a missing tooth, supported by the teeth on either side. The artificial tooth in the gap is the pontic; the crowns fitted over the supporting teeth are the abutments. All three are made as one piece and cemented.

That is where the name comes from: the artificial tooth is suspended between two supports like a bridge. It has no contact with bone at all; it transmits chewing force into the natural teeth on either side.

That is both the bridge's strength and its limit. Its strength is that it is fixed and completed quickly. Its limits are two, and we state both plainly: the supporting teeth are prepared, and the bone under the gap continues to resorb.

The most common bridge is a three-unit design: one missing tooth, two supports. Longer bridges are possible depending on the soundness of the abutments. The general rule is that the span carried should be proportionate to the root surface of the supporting teeth; bridges beyond that ratio are short-lived.

The Real Cost of a Bridge: the Supporting Teeth

We write this plainly because it is so often skipped: a bridge does not protect the neighbouring teeth. On the contrary, it permanently alters them.

The supporting teeth are reduced by one to two millimetres from every surface to receive crowns. That exceeds the thickness of enamel and enters dentine. Where the tooth was sound, that is an irreversible cost whose only justification is closing the gap beside it.

Two risks follow from preparing a tooth. The first is the pulp: in a proportion of prepared teeth, root canal treatment becomes necessary over the following years. The second is marginal decay: the line where the crown meets the tooth is the hardest area to clean, and under a bridge that risk rises.

A bridge does have a genuine benefit, and this is the accurate version: closing the space prevents the neighbouring teeth tilting into it and the opposing tooth over-erupting. That is a real and defensible gain — "it protects the teeth" is not.

The decision point is this: where the supporting teeth already need crowning — heavily filled, root-treated or discoloured — a bridge is a very sensible choice, because closing the gap is added to work that was needed anyway. Where the supporting teeth are sound, an implant should be assessed rather than cutting down two healthy teeth.

Types of Bridge

There is no single bridge design; which one suits depends on the position of the gap, the condition of the supporting teeth and your bite.

The adhesive bridge is the least invasive option and is recommended less often than it deserves. It is held by thin wings bonded to the back of the supporting teeth, with almost no reduction. It is particularly valuable in younger patients as an interim solution until growth is complete. Its limit is that it can debond where occlusal forces are high.

A cantilever bridge is supported from one side and therefore applies a leverage force to that abutment. It works well over short spans with a favourable bite; over longer spans it overloads the support.

An implant-supported bridge spans between two implants and touches no natural teeth. Where three or four teeth are missing, two implants and a bridge achieve the same result as individual implants with less surgery.

On materials, zirconia is the most common choice: with no metal substructure there is no dark line at the gum margin, and it has the strength to carry longer spans.

This is the most common question for a single missing tooth, and the answer varies from mouth to mouth. Putting them side by side makes the decision easier.

On the lifespan of a bridge, let us be accurate. A well-made bridge serves well past a decade — around nine in ten are still in place at ten years and roughly three-quarters at fifteen. But unlike an implant it is expected to be remade at some point, and each remake tends to be larger than the last, because a little more is lost from the supporting tooth each time.

That is the implant's long-term advantage: once placed, it does not commit the neighbouring teeth. Against that it requires surgery, adequate bone and months of healing.

A practical rule: if the supporting teeth already need crowning, a bridge; if they are sound, an implant. There are legitimate reasons that override it — insufficient bone, medical constraints, not wanting surgery.

At the consultation both options should be explained with their timelines, their costs and their limits. A plan describing only one has not offered a choice.

How the Treatment Proceeds

  1. Examination and planning. Radiographs assess the roots of the supporting teeth and the bone level. Where root treatment or gum treatment is needed, it comes first.
  2. Preparation. The abutment teeth are prepared under local anaesthetic. You do not feel pain during the procedure.
  3. Impression or digital scan. With an intraoral scanner no impression material is needed.
  4. Temporary bridge. The prepared teeth are never left exposed; you speak and eat with a temporary.
  5. Try-in. Colour, shape, the relationship of the pontic to the gum and the occlusion are checked. A result you dislike is remade within the same visit — our laboratory is in the building.
  6. Cementation. The bite is checked a final time, excess cement is removed carefully, and cleaning under the pontic is practised with you.

The whole sequence usually takes five to seven working days. For patients travelling within the city the same stages run as separate appointments.

One thing is checked with particular care at the try-in: how the pontic meets the gum. It should touch lightly without pressing. A pontic pressing too hard creates persistent inflammation there; one that does not touch at all traps food beneath it and lets air escape during speech.

The most critical detail at cementation is removing every trace of excess cement. Cement left beneath the gum is the most common and the quietest cause of gum inflammation appearing years later.

Cleaning Under the Pontic

For a bridge patient this is the most important section on the page. Failure to clean under the pontic is among the leading causes of bridges being lost.

The problem is straightforward: the pontic is not attached to bone or gum, it rests above them. There is a narrow space beneath it, and that space is cleaned neither by ordinary brushing nor by floss — floss cannot pass under the bridge, because the three units are one piece.

Left uncleaned, plaque accumulates there with two consequences: decay at the margins of the supporting teeth and persistent gum inflammation in the area. Both progress silently; the patient usually notices when the bridge becomes loose.

  • Super floss. A special floss with a stiff end and a spongy middle. The stiff end is threaded under the bridge and the spongy section wipes beneath the pontic. Once a day.
  • A floss threader. Lets ordinary floss be passed under the bridge.
  • Interdental brushes. More practical where the space allows.
  • A water flosser. Helpful, but it does not replace mechanical cleaning.

We practise this technique with you at the fit appointment. Explaining is not enough; it has to be done together once. Most patients sent away without practising it never do it at all.

Six-monthly check-ups are not optional for a bridge patient. What is examined is not the bridge itself but the margins of the supporting teeth and the gum beneath the pontic. Marginal decay caught early is a small procedure; left late, the supporting tooth is lost and the bridge with it.

Bone Resorption and Appearance

This is the quiet limit of a bridge and it usually goes unmentioned: the bone under the gap continues to resorb.

The reason is simple. What maintains bone is the stimulus a tooth root transmits into it during chewing. When the tooth is removed that stimulus disappears and the bone resorbs markedly in the first year and more slowly afterwards. Because a bridge transmits force to the neighbouring teeth, no stimulus reaches the bone in the gap.

That has two consequences. The first is visible: over the years a space opens between the pontic and the gum. At the front that becomes noticeable and can let air escape during speech; at the back it traps food.

The second is prospective: if an implant is wanted in that site fifteen years later, the bone volume may be insufficient and grafting may be needed. Choosing a bridge today can make tomorrow's implant harder.

We write this not to disparage bridges but to show the whole decision. For a patient whose supporting teeth already need crowning, a bridge remains a very sensible choice; for a younger patient with sound teeth, these two limits weigh heavily.

Aftercare and Longevity

What determines the lifespan of a bridge is not the bridge but the health of the supporting teeth. Ceramic does not decay; the abutments do.

  • Clean under the pontic once a day — with super floss or a threader. The most important item on the list.
  • Brush twice daily, paying particular attention to the crown margins.
  • Interdental cleaning — for the surfaces adjacent to each end of the bridge.
  • A check-up and professional clean every six months.
  • A night guard if you grind. Particularly important with longer spans.
  • No biting hard objects. Ceramic is hard but it does not flex.

The most common problem with a bridge is marginal decay on a supporting tooth. Caught early it can be dealt with without removing the bridge; left to progress, the bridge has to be sectioned off and remade. A little more of the abutment is lost each time a bridge is removed, so an early check-up means a smaller job later.

The second common problem is the bridge loosening as the cement fails. If it feels like it is moving, do not wait: saliva and bacteria getting underneath decay the supporting tooth quickly. A loose bridge caught early can usually be recemented as the same bridge.

What it covers and what falls outside it — accidental damage, neglect, missed recommended check-ups, not cleaning under the pontic and not wearing a prescribed night guard — is set out in writing.

If You Are Travelling for Treatment

A bridge suits a patient travelling for treatment: it is completed in a single visit, usually in five to seven working days. The two visits and the three to six months of healing that an implant requires do not apply.

That convenience should not change the decision itself. The reason to choose a bridge is that the supporting teeth already need crowning, not that it is quicker. Cutting two sound teeth because it finishes in one visit pays for short-term time with a permanent cost. We discuss that plainly at the consultation.

Planning starts before you travel: a preliminary assessment is made from the radiographs you send and we tell you how many days are needed. The definitive plan is written after the examination, because the state of the abutment roots cannot be known without radiographs.

Once you are home, maintenance runs where you live. You are given the name of the material used, the design of the bridge and written instructions for cleaning under the pontic; handing those to your own dentist keeps the follow-up unbroken.
